Registration Information and Applications

FOUNDATION DOG
REGISTRATION

Use the Foundation Dog Registration Application
if one or more of the following conditions exist:

• Your dog’s parents are not registered with us.

• Only one of your dog's parents are registered with us.

• Your dog is registered with another organization as another breed.

LITTER
REGISTRATION

Use the Litter Registration Application if both parents are registered with HMDR.
Puppies from HMDR registered dogs are required to be litter-registered to ensure accuracy.

It is recommended that the breeder of the litter register the litter within 5 weeks of their birth.

REGULAR
REGISTRATION

Use the Regular Registration Application for:

• Transferring a puppy who has HMDR litter registration to regular registration.

• Registering a dog that was born before 10/1/23 whose parents became registered after they were born.
A Breeder Certification Statement is required from the breeder of your dog that verifies the dog's sire, dam, and date of birth.

REVERSE PEDIGREE

REGISTRATION

If the dog you want to register offspring is registered with HMDR, please fill out the Reverse Pedigree Registration Application.
